The membership requires a six-month commitment and a signed financial agreement (promise to pay) at enrollment; the monthly fee is billed each month of the term. “Unlimited visits” means 30-minute medication-management visits as clinically appropriate. Controlled medications will be prescribed only after Northform’s DEA registration, now pending, is approved. Lab fees, level checks and GeneSight or other pharmacogenomic testing are billed separately by the lab; call your lab to confirm pricing. Medications are paid at your pharmacy. Visits and services are subject to availability and clinical appropriateness. The membership is not insurance and is not for emergencies: call or text 988, or 911. Insurance billing is not available yet; eligibility checks will open when Northform joins insurance networks.

Sliding scale
Reduced rates based on need, including for foster parents and youth. Ask during your fit call; you won’t be asked to explain more than you want to.
