Choose a free 10–15 minute fit call, or book your Initial Consultation, a 60-minute psychiatric evaluation ($250), directly. Your intake forms follow when you book, and your Good Faith Estimate arrives before your first visit.

Your planPrescribing and 30-minute medication-management visits ($150), adjusted as you grow.

Northform can prescribe non-controlled medications now. DEA registration is pending, so controlled medications, such as stimulants for ADHD, will be prescribed only after it is approved.

Monday–Saturday, by appointment only. Closed Sunday.

Telehealth

Video visits for patients located in Washington at the time of the visit. How telehealth works.
